Why it happens

Why surgery depletes your iron

Every surgical procedure involves some blood loss. For straightforward operations, this may be modest. For major surgery, such as bowel resection, joint replacement, gynaecological surgery, or caesarean section, the blood loss can be substantial.

When blood is lost, iron is lost with it. The body then needs to replenish both its red blood cell volume and its iron stores simultaneously. This process takes months without intervention, and during that time patients feel exhausted, breathless, and slow to heal.

Recovery from surgery is energy intensive. Your body is repairing tissue, fighting infection risk, and rebuilding. All of these processes require adequate iron. When iron levels are low, everything slows down.

What NICE recommends

NICE guidelines on blood transfusion (NG24, updated 2026) recommend iron supplementation for patients with pre-operative anaemia and support its use in the post-operative period. IV iron is specifically recommended when oral iron is not appropriate or when a faster response is needed. You do not need to accept a slow recovery.

How it feels

Signs of post-surgical iron deficiency

These symptoms following surgery are often accepted as a normal part of recovery. In many cases they are, but when they persist beyond the expected timeframe or feel disproportionately severe, iron deficiency is worth investigating.

Prolonged fatigue

Exhaustion that extends well beyond the expected surgical recovery period. Feeling unable to regain normal energy levels.

Breathlessness

Getting out of breath with minimal exertion. A sign that haemoglobin has not yet recovered sufficiently.

Slow wound healing

Iron is essential for collagen synthesis and tissue repair. Low iron can slow the healing process measurably.

Poor concentration

Difficulty thinking clearly, low motivation, and mental sluggishness are all common with iron deficiency anaemia.

Pale skin

Looking notably pale and washed out. A visible sign of low haemoglobin in the weeks after surgery.

Inability to exercise

Unable to begin rehabilitation or light exercise as recommended, because the body simply does not have the capacity.

Who qualifies

Are you eligible?

Every patient is assessed individually by our GPs. The following criteria apply as a guide for post-surgical patients.

Iron deficiency anaemia confirmed following surgery

Blood tests showing low ferritin or haemoglobin in the context of recent surgery and significant blood loss.

Significant intraoperative or postoperative blood loss

Major surgical procedures where blood loss was substantial, including bowel surgery, joint replacement, caesarean section, and gynaecological surgery.

Oral iron not appropriate or insufficient

Following abdominal or bowel surgery, oral iron is often poorly tolerated and poorly absorbed. IV iron avoids the gut entirely.

Clinical need to optimise recovery

If slow recovery is significantly affecting your rehabilitation, return to work, or quality of life, restoring iron levels promptly is clinically appropriate.
